You can now fill out the PDF forms below with your computer. Steps:

1. Open form and click "Save As..." or "Save and Copy" from the File menu and save to your computer.
2. Close the web version of the form, open the newly saved form on your computer.
3. Fill it out and save it as PDF (this should be the default).
4. You can also Print to options like "Adobe PDF" or "Acrobat Distiller" but you will lose the auto-fill functionality.
5. If you're not sure who to send a form to, call 952.933.9020 or visit our staff page.
6. If you are not comfortable with using digital signatures, print the completed form and sign it by hand before faxing it to 952.933.9021 or sending/delivering it to us at 5750 Lincoln Drive, Edina, MN 55436. Also, let us know if you have any problems.
